Your Role in Supporting Independence

Job Description:
This is an exciting opportunity for someone who is passionate about working with people with disabilities and is looking to make a real difference in their lives. As a Support Worker, you will work closely with individuals to develop and implement personalized support plans, assist with daily living activities, and facilitate community participation. You will also have the opportunity to work collaboratively with healthcare professionals, family members, and other stakeholders to ensure seamless care coordination. Responsibilities:
  • Develop and implement personalized support plans in collaboration with individuals, their families, and healthcare professionals.
  • Assist with daily living activities, such as meal preparation, personal care, and household chores.
  • Facilitate community participation, including social events, leisure pursuits, and education/training programs.
  • Document and report any changes or concerns related to an individual's health, well-being, or safety.
  • Ensure accurate and timely recording of information, adhering to relevant policies and procedures.

Requirements:
- Strong communication and interpersonal skills to build trust and rapport with individuals and their families. - Ability to work independently and as part of a team to ensure comprehensive support is provided. - Previous experience in a healthcare setting, including experience with disabilities and challenging behaviors, is preferred. Experience working with youth is advantageous. - Full driver's licence and access to a vehicle is preferable due to the need to transport individuals as required. - Empathy, respect, and cultural awareness is essential in this role as the individuals we support come from diverse backgrounds with varying needs and interests.
