**About the Opportunity**

This is an exciting opportunity to join our multidisciplinary team as a Mental Health Case Manager with our West Adult Community Mental Health Service. In this role, you will provide expert case management in partnership with clients, their families/whānau, and carers, with a strong focus on returning clients to the primary health care sector following specialist input.

Your key responsibilities include:

  • Oversight and management of a varying case portfolio
  • Completing assessments of consumers and formulating interventions to meet individual needs in collaboration with our multi-disciplinary team (MDT)

To be successful in this role, you will require:

  • Experience in mental health, preferably in a community-based setting
  • The ability to triage and prioritize patients appropriately in a timely manner
  • A minimum of 2 years' experience in a mental health setting
  • A current NZ driver's license
  • Registration with the Nursing Council of New Zealand with a current APC and mental health scope of practice

Our service provides assessment and treatment services for consumers with mental illnesses aged between 18-65 years. We work closely with other services to achieve the best possible outcomes for our clients.
