Job Summary

We are seeking a Home-Based Clinical Services Provider to join our team. As a provider of intensive family therapy, you will work closely with children and families in their homes to promote a healthy and safe environment.

Your key responsibilities will include developing treatment plans, providing intensive family therapy, and coordinating care with other providers.

Essential Duties
  • Develop and implement treatment plans in collaboration with families.
  • Provide intensive family therapy to promote a healthy and safe environment.
  • Coordinate care with other providers, including outpatient therapists, primary care physicians, schools, inpatient hospitals, etc.
  • Complete all required documentation and progress notes.
  • Provide skills training and coaching to children and families in both crisis and non-crisis situations.
Requirements
  • Masters Degree in social work or clinical mental health counseling, or PsyD/PhD.
  • Degree must be eligible for LMHC or LICSW.
  • At least one year of experience working with children, adolescents, and/or families.
  • Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport members.
  • Excellent written and verbal communication.
  • Familiarity with standardized documentation processes.
  • Ability to develop and maintain positive working relationships with employees and providers.
  • Enthusiastic, motivated approach and exceptional interpersonal skills.
