About the Role: As a Care Coordinator with Access Community Health, you will play a vital role in organizing home-based care and support services for our clients. Your primary goal is to enable them to live as safely and independently as possible.

- Roster support workers across the region to ensure seamless coverage.
- Coordinate with support workers to cover shifts and respond to client needs.
- Engage with clients to understand their requirements and preferences.
- Assist with internal queries and contribute to the smooth operation of the team.

- General computer literacy and the ability to multitask under pressure.
- Creative and strategic problem-solving abilities.
