Overview of the Role
The Home Health Licensed Practical Nurse (LPN) plays a vital role in providing personal care and routine skilled nursing services to patients in their homes under the direction of a Registered Nurse. This position requires strong communication skills, attention to detail, and the ability to work collaboratively as part of a multidisciplinary team.
Key Responsibilities
* Administer medication and provide wound care treatment as per physician orders
* Perform vital signs and head-to-toe patient assessments during visits
* Collaborate with physicians, patients, and their families to implement and follow a patient-specific care plan
* Provide education to patients and their caregivers on self-care techniques and information pertinent to diagnosis and treatment
* Document patient information accurately and timely consistent with the plan of care
Requirements
* LPN Program Graduate
* Valid DL with no more than 3 moving violations
* Proof of current/valid car insurance
* Knowledge and experience with electronic health records
* Basic Life Saving Certification (BLS)
