Care Coordination Team Leader

We are seeking an experienced and motivated individual to fill the position of Care Coordination Team Leader. As a key member of our team, you will be responsible for providing leadership and operational management to our Care Coordination team.

Key Responsibilities

  • Provide leadership and direction to the Care Coordination team
  • Develop and implement policies and procedures to ensure high-quality care delivery
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Monitor and evaluate team performance, identifying areas for improvement

Requirements

  • 5+ years of experience in a clinical or healthcare leadership role
  • Postgraduate qualification in a relevant field (e.g., nursing, healthcare management)
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
