Role Overview

We are seeking an experienced pharmacist to join our interdisciplinary team in the Community Health Services Northern Region. This role has been established as part of the community restructuring.

Key Responsibilities

  • Providing clinical pharmacy services to community patients including medicines reconciliation, clinical reviews, optimising therapy, therapeutic drug monitoring, medication adherence assessment, medication counselling and discharge facilitation.
  • Ensuring evidence-based, safe and effective use of medicines and patient-centred care.
  • Contributing towards/leading quality improvement and medication safety initiatives.
  • Continuing to grow your clinical expertise and contributing to education and training of your colleagues and other staff.

Requirements

  • New Zealand Registered Pharmacist with current APC or eligibility to obtain one.
  • At least 5 years relevant experience.
  • Motivated and positive attitude.
  • Excellent communication and interpersonal skills.
  • Prior hospital and/or experience with established clinical pharmacy competencies and technical ability.
  • Relevant postgraduate qualifications in clinical pharmacy (preferred).
