Huntington's Disease Care Specialist

We are seeking an experienced Huntington's Disease Care Specialist to join our team at NZ Health Group. As a critical member of our care coordination team, you will work closely with individuals diagnosed with Huntington's disease and their whanau to provide seamless access to health and disability support services.
Your primary responsibilities will include:
- Collaborating with healthcare professionals to ensure timely and effective interventions.
- Developing and maintaining person-centred shared plans with individuals and their whanau.
- Providing education and support to individuals and their whanau regarding HD progression.
- Maintaining accurate and up-to-date client records and service utilisation data.
