We are seeking an experienced travel nurse to join our team as a home health care specialist. About the Role
This position requires the ability to work variable days, including alternating weekends, and must be willing to live at least 100 miles from the facility. You will be responsible for assessing patient needs, developing care plans, and implementing interventions to improve patient outcomes. Your experience working with high-risk patients, including those with HF, COPD, MI, pneumonia, stroke, and post-op care, is essential for success in this role.

Responsibilities
As a travel nurse in this position, your responsibilities will include:

  • Providing skilled home care services to patients in their homes
  • Assessing patient needs and developing care plans
  • Implementing interventions to improve patient outcomes
  • Working with patients who have high-risk diseases, including HF, COPD, MI, pneumonia, stroke, and post-op care

Requirements and Qualifications
To be successful in this role, the candidate must have:

  • A valid Florida license
  • At least one year of experience as a registered nurse
  • Experience in home health care, including OASIS training
  • IV skills and wound care certification
  • General medical diagnosis and high-risk disease management skills
