Community Health Care Professional

We are seeking a skilled and experienced Registered Nurse to join our District Nursing Service. The successful applicant will provide high-quality patient-focused care to patients in their homes, working across both rural and urban settings within the Hamilton & Rural Family Health team.
**Key Responsibilities**- Provide advanced assessment, management, support, and education for wound management, compression therapy, IV management, palliative/end of life care in the rural setting, continence assessment, catheter management, ostomy management, assessment for acute personal care and home help, support with home oxygen/respiratory patients, rheumatic fever secondary prevention prophylaxis, and liaison/referrals with other health professionals.
- Work effectively within a team, demonstrating sensitivity to cultural aspects and applying the principles of Te Tiriti o Waitangi to practice.
- Utilize excellent assessment skills and time management to deliver comprehensive nursing services to patients throughout their lifespan.
