About the Role

We are seeking an experienced Registered Nurse to join our friendly and cohesive team. As a Clinical Care Specialist, you will contribute to providing expert nursing care to patients and work professionally with all members of the multidisciplinary team (MDT). Your role will involve working as part of a 24/7 roster which includes weekend and night shifts.

Key Responsibilities

  • Provide high-quality nursing care to patients in a community setting
  • Collaborate with the MDT to develop and implement individualized care plans
  • Work autonomously and lead small teams as required
  • Engage with patients, families, and caregivers to promote health and well-being

Requirements

To be successful in this role, you will need:

  • An experienced Registered Nurse with current registration
  • Excellent assessment and communication skills
  • Ability to work autonomously and lead small teams
  • Strong interpersonal and time management skills
