Job Overview

Mahitahi Hauora, a primary health entity underpinned by Te Tiriti o Waitangi, seeks an experienced Registered Nurse to join our Practice Facilitator team. The ideal candidate will possess strong clinical knowledge and experience in supporting General Practice and Māori Health providers in delivering healthcare services that improve patient and whānau health outcomes and access to services.

The successful applicant will work collaboratively with community, primary healthcare providers, and key partners to support practices and providers across primary healthcare programs, improvement methodologies, and equity strategies. As an RN, you will also support workforce capability and development in managing new program rollouts and improving referral pathways, multidisciplinary teams, and access to services that ultimately improve health outcomes for our Tai Tokerau enrolled population.

Key Responsibilities

  • Support practices and providers across primary healthcare programs, improvement methodologies, and equity strategies.
  • Contribute to the development of workforce capability and capacity in managing new program rollouts and improving referral pathways, multidisciplinary teams, and access to services.
  • Foster relationships with healthcare professionals and stakeholders to achieve better health outcomes for our Tai Tokerau enrolled population.

Requirements

  • Be a Registered Nurse with a minimum of 3-5 years postgraduate experience.
  • Have proficient level or above on PDRP or demonstrable experience.
  • Hold an understanding and preferable experience in primary/community healthcare.
  • Be experienced in facilitation, change management, and education.
  • Demonstrate ability to develop and manage relationships with healthcare professionals.
