As a Disability Support Specialist at Community Connections, you will play a critical role in ensuring the health outcomes of people supported by our organisation.

You will be responsible for supporting the development and transformation of organisational and front-line staff practice to ensure continual improvement in the health outcomes of our clients.

This involves striving to achieve the best possible quality of life and choice for individuals with disabilities, through ensuring our health practices are consistent with statutory requirements and contractual compliance.

You will identify health risks and complete individual health reviews to inform ongoing care and referrals to relevant external health providers.

">

  • Registered Nurse with current Annual Practising Certificate (Registered Nurse - preferably with experience working with Intellectual Disability)">
  • Minimum two years post-registration experience">
  • High level of integrity and professionalism">
  • Ability to maintain confidentiality and discretion">
  • Strong verbal and written communication skills">
  • Willingness to travel, which will be required as part of this role">
