About the Job:

We are looking for a highly skilled and motivated Registered Nurse to join our START team as a Nurse Specialist - START Program. In this role, you will be responsible for developing and implementing individualized care plans that promote hospital avoidance and earlier discharge.

You will be working closely with an interdisciplinary team to ensure that our patients receive the highest level of service. This is an exciting opportunity to make a real difference in the lives of our patients.

  • You will have a minimum of 5 years nursing experience.
  • You will have experience in leading and coordinating a team.
  • You will have excellent communication and advocacy skills.
