We are seeking a skilled Mental Health Case Manager to join our multidisciplinary team. As a key member of the West Adult Community Mental Health Service, you will provide holistic case management in partnership with clients, their families/whanau, and carers.
Key Responsibilities
- Manage a caseload of clients with mental illnesses
- Work collaboratively with the multi-disciplinary team (MDT) to develop and implement care plans

Requirements
- Experience in mental health, ideally in case management
