Job Description
As a Community Facilitator, you will play a key role in helping individuals with disabilities to achieve their goals and aspirations. This includes providing support to help individuals develop their skills and abilities, living independently in their chosen home, and enhancing inclusion in their community.
Key Responsibilities
- Develop individualized plans to support the goals and aspirations of individuals with disabilities
- Provide support to help individuals develop their skills and abilities
- Support individuals to live independently in their chosen home
- Enhance inclusion in the communities of choice
- Assist and support employment opportunities as they arise
Requirements
To be successful in this role, you will require:
- Some experience supporting or working with people with disabilities
- A NZ Certificate in Health & Wellbeing (Support Worker) Level 3 or 4, or an equivalent qualification
- The ability to work autonomously and be part of a team
- The ability to be flexible with working hours and approach
- The ability to think on your feet and use initiative
- An open mind and a willingness to learn
- A calm nature with a sense of humour
- A reliable vehicle
- A current first aid certificate would be an advantage
- Completed or working towards a Bachelor's degree or equivalent in Education, Psychology, or a related field would be an advantage but not essential
- Good written and verbal communication skills
