**About the Role:**

We are seeking a highly skilled and experienced Diabetes Specialist Nurse to join our nationwide team of full-time advisors working in primary care.

The successful candidate will be responsible for managing diabetes within primary care, including identifying sub-optimally controlled patients, optimizing oral therapy, conducting point-of-care testing, and referring for specialist insulin treatment where necessary.

This is an excellent opportunity for a motivated and experienced nurse to make a real impact in Diabetes care and contribute to our nationwide team's success.

Key Responsibilities:

  • Managing diabetes within primary care, including identifying sub-optimally controlled patients
  • Optimizing oral hypoglycaemics and conducting point-of-care testing
  • Referring for specialist insulin treatment where necessary
  • Exercising corporate nursing responsibilities
  • Providing education, support, and advice to people with Type 2 Diabetes

Requirements:

  • A minimum of 3 years experience (post-registration) managing diabetes within primary care
  • Knowledgeable and confident in the provision of education, support, and advice to people with Type 2 Diabetes
  • Skills and knowledge in the optimisation of oral hypoglycaemics
  • Full driving license
  • Current NMC registration
  • IT literacy, to include primary care systems
