STATEMENT OF PURPOSEThis role supports and assists the nursing team and the medical social work team in administrative tasks, information gathering, form filling, fieldwork, and other ad hoc duties.KEY RESPONSIBILITIESPre-admission/Admission SupportOrientate new residents and their NOKs to the nursing home.Ensure residents settle well in the nursing home.Casework (non-complex case)Engagement and AssessmentEstablish rapport and build professional relationships with residents and significant others.Identify and clarify the key issues to the problem situation and gather relevant information from residents and NOKs.Conduct bio-psycho-social-spiritual assessments to identify severity, key risks, and protective factors for intervention.Engage residents, families, multi-disciplinary teams, and relevant stakeholders to gather holistic and relevant information.Goal Setting and Intervention PlansAnalyse and evaluate different courses of action with the multi-disciplinary team and relevant stakeholders.Formulate appropriate strategies for action.Conduct care planning and goal setting with residents based on standard guidelines and professional assessments.Set clear contracts and goals with residents to engage them in problem-solving.Apply appropriate assistance schemes.Collaborate and negotiate with stakeholders to mobilise community resources.Case Review and DocumentationFollow up and review care plans, providing recommendations for modifications where necessary.Evaluate the process and outcomes objectively and recommend future improvements.Provide documentation and timely updates of assessments and interventions for multi-disciplinary professions.Work with MSWs on providing social reports with proper consent to relevant stakeholders to ensure continuity of care.Keep residents meaningfully engagedUnderstand the interests of individual residents.Organise activities that are of interest to residents.Engage and motivate residents to participate in activities.Work with volunteers and the Volunteer Manager in activities planning and implementation.Review and evaluate the effectiveness of activities in improving residents' well-being.Support volunteer programProvide feedback on possible new volunteering opportunities based on observed needs.Work closely with the Volunteer Manager to effectively mobilise resources for the volunteer program.Activities ProgrammeProvide supervision during the implementation of activities.Provide feedback to volunteers on areas of improvement via Corporate Communication in charge.Support Advance Care Planning (ACP) programInitiate ACP awareness with residents and their NOKs.Facilitate General ACP or PPC with residents and their NOKs for those who are ready.Review ACP plans regularly.Assist residents in fulfilling their care preferences and last wishes.ADDITIONAL DUTIESThe staff will undertake projects relevant to the post, as requested by HOD or ED.
