Job DescriptionHabit Health is a nationwide healthcare provider, specialising in the rehabilitation space.
We enable people to live their best lives and empower our teams to realise their full potential. Our Hamilton clinic is looking for an enthusiastic Rehab Coach who will empower our clients to regain their autonomy and participation across the demands of everyday life.
We work hard to ensure our communities live their best lives.
Main responsibilities:
• Work with our therapists to execute TI plans
• Link the client to community resources / support
• Utilise diary management software and guides
• Ensure client queries are communicated to the leading therapist

The ideal candidate:
• Maintain relevant board registration / APC where required
• Ability to effectively manage your time and caseload
• Full NZ Driver's license for community rehabilitation
• Excellent communication skills – you will be meeting new and varied people every day
