In every aspect of our work, we are committed to helping whanau be the best they can be.In this role, you'll work closely with Health and Wellbeing Practitioners based in GP practices, including Doctors, Health Improvement Practitioners, Health Coaches, Nurse Practitioners, and Social Workers.
Your focus will be on supporting tangata whaiora with community-based solutions tailored to their unique needs in the low to moderate mental health and addictions space.
Access & Choice is an easily accessible, responsive service that provides practical and meaningful support for immediate needs and barriers to wellness.
No two days will be the same—you might be empowering someone to build confidence in managing their health, supporting a client experiencing social anxiety and connecting them to community resources, assisting with housing or social services needs, or guiding someone through mental health or addiction challenges while connecting them with long-term support.You'll be surrounded by a strong support network, both internally and externally, with access to a range of services and initiatives to ensure the best outcomes for those you work with.
You will support the service manager with the external and internal relationships promoting partnerships aligning to a tangata whaiora-centered approach.This is a permanent, full-time role (Monday to Friday, 40 hours per week, 8-4.30pm) that offers variety, flexibility, and the chance to make a real impact in people's lives.The ideal candidate will bring a compassionate, solutions-focused approach to conflict resolution, along with excellent planning skills to support tangata whaiora in achieving their goals.
Your deep understanding of community services and strong local connections will be key in supporting tangata whaiora to access the right resources at the right time.To be successful in this role, you'll ideally have the following:A Level 4 (or higher) qualification in Mental Health and AddictionPrevious experience within the mental health and/or addiction sectorUnderstanding of Te Whare Tapa Wha/Hua OrangaStrong understanding of Maori and Pasifika cultures is preferredA Full New Zealand Driving LicenseExperience with Microsoft Office products and ideally Client Management systems i.e.
