Employment of Care co-ordinator and Allied Health staff - Expression of InterestThe health system is changing, and Whanganui and district is one of the regions early in the adoption of expanded primary care, with the idea of working alongside general practice and Iwi providers as one team serving one population.The intent for Comprehensive Primary Care Teams (CPCTs) is to provide greater scope and support for people in the health system earlier, address inequalities (cost and access) and support those with the greatest risk of inequalities or admission to hospital. CPCTs will expand general practice teams from a traditional doctor/nurse workforce model to an expanded team of Pharmacists, Physiotherapists, Care Coordinators and Kaiawhina.We are seeking expressions of interest from primary care nurses and allied health professionals who are keen to be a part of this initiative, the interested parties must:Enjoy the challenge of delivering services to those with a range of clinical and social needs;Provide safe effective care for whanauBe culturally competent; andBe able to work effectively within a General Practice team and with external health and social service providers.The service model is being co-designed with Iwi, General Practice CPCT partners and community.
