ABI Rehabilitation, with facilities in Auckland and Wellington, is the lead specialist provider of intensive adult in-patient brain injury rehabilitation in New Zealand. We have attained accreditation with Commission on Accreditation of Rehabilitation Facilities (CARF) International - an international gold standard for person-centred, quality rehabilitation.
A full time, permanent position has become available to join our progressive inter-professional team. This role will involve providing specialist keyworker services in our intensive inpatient services - covering the immediate post-acute stage through to community re-integration and referral to community/out-patient phase.
The applicant must have excellent communication skills, establishing relationships with our stakeholders (clients, families/whānau, ACC, HNZ-Te Whatu Ora, rehabilitation providers, external organizations, educational providers, etc.), excellent organization/ planning skills, and a demonstrated track record of teamwork - to provide specialist services to clients presenting with acquired brain injury.
The case load is varied and key areas include:
- Promoting a smooth and efficient pathway for clients and families/whānau
- from admission to our facility through to discharge
- Ensuring that families/whānau
- are engaged in the rehabilitation process as an integral part of the rehabilitation team
- Monitoring family/whānau
- satisfaction and promoting quality improvements as required
- Developing and implementing family/whānau
- information and education systems
- Managing discharge planning processes and communication from the time of admission
- Overseeing and engaging in clinical advise and support to clients and families post-discharge from ABI as required.
We can offer excellent opportunities for professional development including regular peer reviews, formal supervision, and in-service training. Attendance and participation in national and international conferences / workshops is encouraged in order for ABI Rehabilitation to share clinical expertise with the wider brain injury rehabilitation community.
A minimum of two (2) years-experience working with clients in similar post-acute rehabilitation settings would be preferred. A current practicing certificate or registration with a professional body /organization is essential.
