**location : CHT Hillsborough Care Home**

**Weekend Cover -** 7.5 hours per day.**
Key Accountabilities:

- Contributes to the assessment process of each resident under the guidance of the Unit Manager reflecting the individual resident’s functional level, problems, needs and interests.
- Plans, schedules, implements and evaluates a programme of individual and/or group activities based on the residents' needs.
- Coordinates and participates in resident activities designed to promote social interaction, reality orientation and intellectual stimulation.
- Documents activity-related information in the resident’s notes.
- Assists with escorting residents to and from activities.
- Maintains sufficient activity supplies.
- Develop monthly calendar of planned activities.
- Contributes to the development of the continuous quality improvement plan for the Activity Programme.
- Consistently act as a role model for safe work practices.
- Ensure that all incidents and accidents are reported in accordance with CHT policies and procedures.
- Comply with the Health and Safety at Work Act 2015, relevant legislation and regulations and CHT’s policies and procedures.
- Attend and actively participate in team health and safety meetings.

- Good oral and written communication skills, with clear spoken English
- Good personal presentation

- Possesses good leadership skills
