Te Whatu Ora, Health New Zealand Te Pae Hauora o Ruahine o Tararua MidCentral (MidCentral) provides healthcare services to over 180,000 people across the Manawatū, Horowhenua and Tararua regions. With a large hospital campus at Palmerston North, it also provides key health care and support services to neighbouring communities.

Supportlinks is the Needs Assessment and Service Coordination (NASC) service in the Te Uru Whakamauora (Healthy Aging and Rehabilitation) directorate which provides access to support services in Te Pae Hauora o Ruahine o Tararua MidCentral district for people with acute/short term needs, long term conditions, older people and Kaumātua, and mental health and addictions.

Supportlinks works primarily with a community focus however also has a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged.

Supportlinks is part of the new and exciting team within the Older People's Assessment and Liaison Community Service (OPAL CS) which compliments the existing range of services provided to older people with a focus on reducing avoidable displacements from home. This service aims to shift more health care from hospital settings closer to people's homes, and pivotally, incorporate Kaupapa Māori approaches in its service to achieve equitable outcomes for Māori kaumatua.

Do you have?
- An ability to work within a changing health environment
- Excellent written, oral and interpersonal skills with an ability to communicate with people from a wide variety of backgrounds and cultures
- An ability to motivate and support behavioural change within complex environments
- Proven skills as an educator and role model
- An ability to successfully facilitate group work and confidence with making presentations to large groups
- A mana enhancing approach within your practice
- Strong organisational and time management skills
- A high level of computer literacy and confidence to use a laptop and Microsoft Office programs including Word, Outlook, and Powerpoint
- A valid full NZ drivers licence
