**About THINK Hauora**
THINK Hauora is one of the largest primary health organisations (PHO) within New Zealand. We have up to 140 employees, clinical and non-clinical, striving to improve wellbeing and health outcomes across the MidCentral region. **The Here Toitū Programme**:
The Ministry of Social Development (MSD) and Primary Health Organisations (PHOs) have partnered to deliver “Here Toitū” - a national programme that supports people living with a health condition or disability move towards employment, or those already in employment who have health concerns that are impacting on their ability to work. THINK Hauora is one of four PHO’s working in partnership with MSD to deliver the Here Toitū programme in different regions across the country.
**The Role of Kaimanaaki**:
The Kaimanaaki (Health Navigators) are part of a dedicated team that supports whānau on their wellbeing journey as part of the 12-month programme. Using a strengths-based approach, you will assist whānau to identify goals and aspirations, explore ways for them to achieve their desired outcomes, whilst overcoming any barriers. Our Kaimanaaki are responsible for engaging with GP teams, MSD Case Managers, employment specialists and whānau to deliver supportive interventions.
This full-time, permanent role is based centrally in Palmerston North. Daytime travel within the Mid-Central region is required, and you will have access to one of the of company lease vehicles. A full, clean driver’s licence is essential.
Supported by a Programme Lead, and the wider Here Toitū team, your role will involve:
- Assessing whānau for suitability for the programme
- Supporting whānau to improve their health and independence, promoting self-management
- Working with whānau to develop a wellbeing action plan and take actionable steps
- Maintaining and enhancing active working relationships between providers, communities and whānau, hapu and iwi to strengthen a whanau-centred approach
- Assisting whānau to reduce or overcome any barriers to accessing and navigating support so they achieve their goals and aspirations
Ultimately, the service aims to foster the pathway of knowledge to strength, independence, and growth for future generations.
We would love to hear from you if you have:
- A relevant tertiary qualification (e.g., Certificate in Health and Wellbeing level 3 or above) and/or work experience in a similar health care or community setting
- Experience of working in a whānau-centred way
- Proficiency in Microsoft Office and patient/client management systems
- Well-rounded interpersonal skills, have compassion, are a natural motivator and enjoy problem-solving
- An ability to understand and effectively engage with people across cultures, in particular Māori and Pacific Island populations. An ability to speak basic Te Reo Māori and/or a Pacific language would be a distinct advantage.
- Demonstrated ability to work independently as well as in a team environment

Salary range: Circa $55K - $65K gross per annum (as assessed against experience in the key competencies)
