Remote Monitoring Nurse, Centralised Services, Locality Community Health Service

We are looking for experienced nurses to be part of an exciting new service providing monitoring to patients admitted under Hospital in the Home (HiTH), within the community. In this role you will gain experience remotely monitoring patients using innovative wearable technology. The role will work as a member of an interdisciplinary team which includes SMO, Nurse Practitioners, Allied Health and HCA staff working across the Counties Manukau Health district.
As a registered nurse you will be remotely monitoring community-based patients to support patient recovery and admission avoidance. The Hospital in the Home team uses a combination of wearable technology and phone triage to undertake patient assessments/interventions with rostered shifts covering 24 hours, Monday - Sunday, 365 days. You will provide support and intervention across a myriad of both acute and long terms medical conditions, including triaging of patients for HiTH and patients in the wider community.
The role requires a range of skills including (but not limited to)
- a strong communicator, as most patient assessments will be completed over the phone,
- advanced clinical assessment skills.
- an ability to work autonomously
- initiating and developing innovative approaches to problem solving
You will be part of a wider community team that interacts with clinicians, other health care providers, and community agencies. Having someone that strives to make improvements, while utilising skills and input from other team members to co-ordinate patient care, is important to us.
You will be empowered to make decisions or recommendations relating to patient management, while demonstrating a high level of communication and professionalism.
The position offers a hybrid working environment that supports a blend of in-office and working from home.
**Essential requirements**:
- Registered with the NZ Nursing Council
- A minimum of 3-5 years recent clinical experience
- Functioning at proficient level on PDRP (preferred)
- Knowledge or experience in acute assessment and care co-ordination
- Background of cardiology and long term medical conditions (preferred)
