**About the Role**
We are seeking an experienced community support worker to work in a new community based team
- Te Whakawhitinga.
**Te Whakawhitinga** is a co-ordinated primary social service aligned to the Te Whatu Ora Waiora vision and underpinned by Kotahitanga. The Kotahitanga values of unity, togetherness, shared vision, working together, trust, relationships, collaboration and integrations are fundamental to the service model of Te Whakawhitinga.
We intend to meet the needs of tangata hāpori/ tangata whaiora across the housing continuum, who are experiencing health, social and environmental issues that could put tenancies at risk or have resulted in homelessness.
We will do this by being a tangata hāpori/ tangata whaiora led service that;
- helps a person identify their goals and aspirations that will achieve mauri ora
- supports access to community resources and services that will meet these goals
- works in close partnership with tangata hāpori/tangata whaiora and other agencies in a transparent and coordinated way.
**About you**
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
- An experienced community support worker.
- Ability to follow direction from registered staff to enthusiastically offer practical support for tangata hāpori/ tangata whaiora to achieve good health and social outcomes.
- Excellent rapport and relationship building skills.
- Whanau centred, collaborative and partnership approach.
- Non-judgemental and empathetic.
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
- Experience working in a mental health, community health or primary care setting.
- Experience working with Māori and Pasifika groups.
- Highly developed interpersonal communication and negotiation skills including demonstrated ability to build productive working relationships with colleagues to achieve required outcomes.
