**He kōrero mō te tūranga | The Role**:
We provide mana-enhancing services that promote health and well-being for individuals, whānau, and communities. We provide support to make positive changes in our clients' lives, by helping them identify their strengths and to help set goals and plan the necessary steps to achieve them. In this role you will be supporting people in their own homes depending on what their needs are. This could be from observing medication to supporting people to re
- integrate back into the community.

This is a permanent full-time position working rostered shifts on any 5 days of the week. This position will be based in Wellington.

**Duties of the role could include, not limited to**:

- Encourage and support Tangata Whaiora connections with Whānau and provide support with appointments around mental and physical health.
- Aid the Transition Process and walk alongside Tangata Whaiora as they learn to live as independently as possible within the community.
- Working collaboratively with internal and external stakeholders
- Support Tangata Whaiora to connect, maintain and build relationships with their own whānau, iwi and community.
- Ensure key documentation and handovers are accurately recorded.
- Attend and contribute to team meetings, bringing ideas and updating others on their activities.

**He kōrero mōu | About You**:
We are looking for someone who is positive, flexible, vibrant, confident, and compassionate to work in our service. You'll use your non-judgmental attitude and belief to motivate and encourage whānau realise their potential. You'll have the ability to easily build professional relationships, while maintaining strong professional boundaries.

**To be successful in this role, you will need**:

- NZ Certificate in Health and Wellbeing - Level 4 (desirable).
- Ability to work collaboratively with mental health professionals and within our team.
- Experience working in a similar role.
- Must be flexible to work around rostered shifts.
- Demonstrated commitment to recovery values.
- Advanced communication and interpersonal skills.
- Eligible to work in New Zealand.
- A current full driver's licence.
