This Clinical Nurse Specialist - Older Adult role is an opportunity to be part of the Patient at Risk of Increased Stay (PARIS) team. You will join another Clinical Nurse Specialist (CNS), an Occupational Therapist, a Social Worker, and a Physiotherapist, as part of the Specialist Health Services for Older Adults at Tauranga hospital.
The role will work within a model that completes Comprehensive Geriatric Assessments, within a supportive team environment to coordinate clinical care, rehabilitation, and discharge planning in both the Emergency Department and Assessment Planning unit. This model is specific to older individuals who have been identified as being at risk of needing extended care, particularly frail older adults with pre-existing functional challenges. The role also contributes to education.
Additionally, the CNS will facilitate the availability of specialised assessments within the community.

- Have significant experience with Older Adults
- Registered as Practitioner with authorised prescribing & hold a current practising certificate
- Postgraduate academic papers in Gerontology would be an advantage
- Able to demonstrate an understanding of our local community and the health needs & aspirations of the people that present to the service.
