**About the Role**

We are seeking four registered health care professionals (Registered Nurse, Occupational Therapist or Social Worker) to join two new mental health community teams.

The primary purpose of the roles is:

- Facilitating tāngata hāpori/ tāngata whaiora transitions throughout the service system.
- Improving the flow of information amongst everyone identified in the tāngata hāpori/ tāngata whaiora’s circle of support.
- Monitoring the systems and processes that underpin the provision of good quality care and support.

Key responsibilities of the roles include:

- Conducting an initial assessment of needs in partnership tāngata hāpori/ tāngata.
- Working with tāngata hāpori/ tāngata whaiora, their whaanau and providers to produce plans together, to best meet their needs and preferences.
- Coordinating the tāngata hāpori/ tāngata whaiora’s plan to direct, enable and evaluate a range of interventions, from a range of providers to meet goals identified.
- Developing and building strong interagency relationship across Iwi, Māori NGO providers, Government organisations, Non-Governmental Organisations and the Mental Health and Addictions Sector.

There are two new services being established with Te Whatu Ora’s Mental Health and Addictions Services:
**Te Whakawhitinga**

Nurture the people “Ahakoa kowai, ahakoa noo whea me whangai I te tangata”

Te Whakawhitinga is a Te Whatu Ora, Waikato co-ordinated primary social service aligned to the Te Whatu Ora Waiora vision and underpinned by kotahitanga. The kotahitanga values of unity, togetherness, shared vision, working together, trust, relationships, collaboration and integrations are fundamental to the service model of Te Whakawhitinga.

Te Whakawhitinga is a tāngata hāpori/ tāngata whaiora led service that will identify and support access to community resources and services to achieve their goals and aspirations. It will enhance the service provision of primary and secondary health and social service organisations, by working across the housing and social care continuum to ensure partner agencies interventions for the tāngata hāpori/ tāngata whaiora ‘My Plan’ are shared, transparent and coordinated to achieve mauri ora.

**Oranga Rangatira**

The service has been named Oranga Rangatira to reflect the mana-enhancing intention. Note that ‘oranga’ in the general sense covers wellbeing, welfare, survival, health and living. The main description for a Rangatira is a first born or high ranking of a senior family in an iwi. However, the characteristics of a Rangatira includes utilising te reo Māori, tikanga, accessing marae, wahi tapu and access to whānau, hapu and iwi. These are all skills that will be offered to whānau under Te Ao Māori, to lift and build their identities, confidence and skills.

This will be an integrated service that is offered to tāngata whaiora in the community and within a residential unit.

You will be part of a multi-disciplinary team (clinical and non-clinical roles) that fosters a culture of mutual respect, safe practice and acknowledges contributions from both cultural and clinical perspectives.

Part of your role will be to support mana whenua to exercise their kaitiakitanga and manākitanga obligations within the context of the service.

**About you**
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
- Whanau centred, collaborative and partnership approach.
- Experience working in a mental health, community health or primary care setting.
- Experience working with Māori and Pasifika groups.
- Highly developed interpersonal communication and negotiation skills including demonstrated ability to build productive working relationships with colleagues to achieve required outcomes.
- Ability to take a proactive role with other organisations to achieve sustainable outcomes for tāngata whaiora.
- Registration with the applicable registration board with a current annual practicing certificate.
