Nelson Bays Primary Health (NBPH) is the Primary Health Organisation for the Nelson Tasman area. NBPH leads and coordinates primary health care in the Nelson Tasman region and is committed to reducing the inequalities in health between all peoples.

NBPH is seeking a health promoter to make a difference to the health and well-being of people in the Nelson Bays region in the following role:
**HEALTHY LIFESTYLE FACILITATOR**

**Permanent part-time position**

**(0.5 FTE - 20 hours per week)**

The position will work within the Green Prescription team and as part of the wider Health Promotion team. The focus is on the prevention and management of long-term conditions by empowering clients to build the knowledge, confidence and self-management skills required to make and sustain long-term healthy choices (or behaviours) for improved health outcomes.

**The successful applicant will have**:

- A current qualification or experience in community health, group fitness or recreational exercise;
- An understanding of health promotion principles, including self-management, health literacy and motivational interviewing skills
- A “people person”, able to relate to diverse populations and communities
- Basic to intermediate computer skills, including use of databases, Microsoft Office
