Te Whatu Ora, Health New Zealand, leads the day-to-day running of
our health system across New Zealand, with functions delivered at local,
district, regional and national levels. It weaves the functions of the 20
former District Health Boards into its regional divisions and district offices,
ensuring continuity of services in the health system.
Mō te tūnga | About the role
Come and join
the Complex Clinical Care Network (CCCN), a team of multi skilled health
professionals. This is a position (1.0 FTE) based West Coast wide.
CCCN invests
in a proactive system of care to wrap services around people identified with
complex needs, to ensure they stay healthy and well in their own communities
for as long as possible. The assessment tool used is the interRAI Home Care.
You'll work
with clients and their families/whanau; and in collaboration with the
inter-disciplinary team to address issues identified in the assessment. As a
central player in the team you'll find out what is needed and see that your
clients receive appropriate and timely support.
CCCN is a
leader in the restorative model for clients with complex needs and is a point
of referral to support delivery of integrated services.

Experience
You will be
an experienced registered nurse or health professional and bring enthusiasm and
a desire to work collaboratively to respond to the broad ranging needs of
patients. Excellent communication, computer and organisational skills are a
prerequisite of this varied role. A current
full New Zealand driver's licence is essential.
