Hospital in the Home (HITH) provides admitted sub-acute patients care in the comfort of the patient's home or other suitable location. Research findings demonstrate that patients have improved outcomes and recovery at home with fewer complications such as infection, delirium, and confusion. Patients can be offered this option if the care they need can be delivered safely in their homes.
The objective is to provide care as close to home as possible. The care is person-centered, of value to the patient, their whanau, and carers, and is respectful and responsive to their preferences, needs, and values, and is culturally appropriate.
The hospital-in the Home service has a new and exciting opportunity for a Nurse Practitioner to join our energetic and friendly team. You will receive SMO collegial support, and work with skilled experienced nurses and the administration team. The expansion of the service is supported by a project management team.
You will need to be able to work autonomously and part of your role will be to conduct physical assessments, diagnose, and treat patients with sub-acute conditions, provide screenings, and prescribe medications to our patients at risk of deterioration in an acute clinic environment. Providing one on one advice to patients means that you will need to be friendly, compassionate, and proactive in the management of conditions.
Most importantly you will have excellent communication and interpersonal skills, and an ability to build a strong rapport with team members.
