St Clair Park Residential Centre Limited is an adult care home that works along side all adult with diverse support needs. Our aim is to assist the people who live here to maintain their independence, be involved in making decisions affecting their lives and to provide opportunities for community participation. We encourage everyone who lives here and their family/whanau to be involved in the planning and provision of support, while ensuring we provide a homely environment. We operate from a recovery perspective that focuses on peoples strengths to enable them set goals that are meaningful, rewarding and achievable. Each person has an appointed Key Worker who helps them to navigate their journey into full-time care and ensures they live the life they choose to live.

Number of Positions: Up to 5 positions.

Employment Type: Permanent.

Days required: Rotating Roster and weekends as required.

Hours per week: 30 to 40 hours per week.

Hourly rate of pay: $28 to $35.

Job pre-requisites: Having work experience and/or a suitable qualification will be beneficial but not necessary as we need workers urgently.

The Employee will perform the following tasks and duties:

- Read assigned residents' notes relevant to the care they require on a daily basis.
- Carry out general cares as required in the resident's individual care plan including supporting independence with activities of daily living.
- Carry out all cares in accordance of St Clair Park Residential Care Policies and Procedures and with the Residents' Code of Rights.
- Observe and report to the L4 caregiver any changes in resident's personal requirements or condition.
- Observe for and report any bruising or unusual marks.
- refers any concerns or complaints to the L4 caregiver.
- Ensure residents are adequately monitored at all times whilst in the facility.
- Facilitate activities as per the activities plan.
- Facilitate residents maintaining current abilities by encouraging in activities of daily living.
- Implement diversional therapy techniques as listed in the individual resident DT plan to provide meaningful activity to the resident's interests and manage challenging behaviour.
- Ensure that resident's rooms maintain an environment of safety including infection control.
- Ensure a clutter free environment with the highest infection control practices implemented.
- Contribute to housekeeping requirements as per task list or assigned requirements including food serving.
- Notify Manager of any maintenance issues using the correct procedure.
- Encourage mobilisation for resident health in a safe manner and in accordance to the resident's care plan.
- Ensure that residents have access to emergency call bells at all times.
- Answer call bells in accordance with St Clair Park Residential Care Policy.
- Respond to incoming phone calls and carry Dect phone as allocated per shift.
- Check the facility for security and hazards throughout shifts.
- Assist residents to meet their hydration and nourishment needs in accordance with their care plans.
- Assist with dishing up and serving of meal in the dining room and to the resident's room. Clean tables after meals.
- Report to L4 Caregiver or RN if any resident has a change in food or fluid intake from that indicated in the care plan.
- Facilitate residents' daily participation in organised or other activities.
- Residents privacy is respected when they have visitors or wish for a quiet time.
- Residents are assisted as necessary to maintain contact with family, friends and the community.
- Train, gain and maintain medical competency to administer medicines using Medimap when delegated to do so.
- Follow St Clair Park Residential Care medication policies and procedures.
- Report any concerns or issues in relation to medications to the L4 caregiver.
- Participate in handover meetings at the beginning and end of each shirt as required.
- Report any issues or uncompleted tasks arising on the shift at the handover that need to be dealt with by the next rostered shift.
- Report issues again that span over more than a 24 hour period.
- Have excellent record keeping and maintain daily progress notes for each designated resident.
- Record bowel habit, fluid intake and restraint monitoring as required for each individual resident.
- Document in the notes any change in the resident's normal health status.
- Direct and implement manual handling procedures correctly utilising manual handling aides and get assistance as indicated in the care plan.
- Use all equipment appropriately and report any defects, breakages and potential problems in the maintenance book as soon as it is noticed.
- Keep cleaning chemicals in view when using them and lock them away when not in use.
