THINK Hauora provides primary and community health services across Palmerston North, Manawatu, Tararua, Horowhenua and Ōtaki communities. We are an integrated health service that supports core alliances with Te Whatu Ora and other key health providers. We are committed to Te Tiriti O Waitangi and Pae Ora.
The Community Connector is a new position. The purpose of this role is to provide expertise and connection to support the implementation of localities across the Midcentral region, within the broader landscape of Whānau Ora and Collective Impact, in conjunction with takiwā locality partnerships and networks, Te Pae Oranga o Ruahine o Tararua, Te Pae Hauora o Ruahine o Tararua and key stakeholders.
Reporting to the Director, Connecting Communities and working collectively with our partners, the role will focus on:
- Partnering with Iwi and Māori organisations and key stakeholders to ensure whānau Māori, Iwi and communities have a strong voice in deciding what’s needed in their local area, their aspirations and priorities for improving hauora outcomes
- Supporting the development and co-design of community led, intersectoral primary and community solutions and enable different health and wellbeing organisations to work collectively to improve people’s experiences of health care and ultimately, improved health outcomes
- Supporting the development and implementation of Locality Plans across the rohe
What we are looking for is someone with proven leadership experience combined with extensive relationship and community engagement experience. You will have experience of working on collective projects with multiple organisations which has resulted in positive change for people, whānau, or communities.
Having operated at a similar level, you will be able to confidently construct and write change management business cases and manage streams of workflow. Colleagues will describe you as highly collaborative, relationship focussed, trustworthy, people focussed, and respectful with the ability to get things done.
