**Support Worker Team Leader - Intensive Support Youth Services**

NZ Community Living is part of Healthcare NZ limited and provides services to people with disabilities across New Zealand; We are committed to providing support, respect and dignity to all New Zealander's enabling them to have greater independence in the community.

We are looking for an energetic and motivated Team Leader to provide support to a male client in his own home. You will lead a team in supporting our clients to live as independently as possible whilst also providing a safe environment where they can develop confidence, build friendships, and interact within the community.

**What does the role involve?**

As a Team Leader within our Intensive Support Services, you will assist youth with disabilities to live as independently as possible within their homes, school and community, as well as leading and motivating your team. You will help the people we support to achieve their goals and dreams and reach their potential, through both practical and emotional support.

This role will include the flexibility to work evenings, overnight, and weekend shifts. Responsibilities can be varied and you will follow a support plan, which may include some or all of the below:

- Embedding Positive Behaviour Support in your work
- Activities at home and in the community that are meaningful and purposeful to the person, including maximising independence and involving Whanau
- Household duties including meal preparation and daily care routines
- Working together with a team to implement personal support plans consistently, with effective and positive teamwork and communication
- Providing people with a warm and nurturing environment to live in, which is inclusive of ideas and feedback so that each person is supported towards their personal goals

**What are we looking for?**
- A desire to support our rangitahi / young people to learn new skills and achieve outcomes
- Previous experience working with young people/rangitahi
- Previous experience supporting mental health
- Creativity 'out of the box' thinking and a team player
- A full NZ driver's licence
