An opportunity has arisen to join the Health of Older People team at Disability Support Link. We are looking for experienced registered health professionals with a current annual practising certificate.

A few key areas of responsibility:

- Working to improve and break down the barriers for Whānau/people accessing services
- Networking and maintaining relationships with key stakeholders

**Mō tātou Ratonga | About our Service**

Disability Support Link is the Needs Assessment and Service Coordination agency for the Waikato region. You will join a small and dedicated team of assessor coordinators. This will be a varied and exciting role working alongside older people, family/Whānau and the wider community (including home based support providers, aged residential care providers, general practitioners, hospital staff and other NGO’s). This position is based in Hamilton.

- Proven assessment and goal setting skills
- Proven ability to work autonomously
- Passion, motivation, and initiative to work alongside older people/Whānau
- Current full New Zealand drivers licence
