Nelson Bays Primary Health (NBPH) is the Primary Health Organisation (PHO) for the Nelson Tasman region. PHO’s lead and co-ordinate primary health care within the local community. NBPH also manages an integrated health facility, Golden Bay Community Health, who deliver a wide range of health services to the local rural community in Takaka.

An exciting opportunity has become available in our Richmond based office for the following role:
**Gateway Coordinator**
**0.5FTE / 20 Hrs per week**
We are looking for an experienced individual with a successful background in high clinical administration and coordination to gain new skills within a committed, friendly team. This position coordinates service delivery for high needs groups, utilising existing systems and processes where possible to develop the capacity and capability of all key agencies.

- Are you self-motivated, driven to ensure our vulnerable whanau are well supported and cared for nurturing relationships with whānau, hapū and iwi
We require the successful applicant to work 20 hours per week (0.5FTE). Applicants must be highly organised, systems orientated, and sound working knowledge of how key agencies work with families in the social, health, education, and youth justice sectors.
