**About the role**

The Adult Community Therapy Service (ACTS) delivers allied health services to adults aged 16-65 years in our Canterbury community. Our team includes Clinical Assessors, who provide needs assessment and service coordination (NASC) for clients funded by Long Term Supports-Chronic Health Conditions (LTS-CHC). This client group includes people aged 0-65 with complex chronic medical needs.

Our focus is the care and wellbeing of clients/whanau. We utilise a restorative model of care, supporting clients to live well within their communities. This can involve co-ordinating a range of services to support living at home, as well as facilitating transitions to supporting living or care facility options.

This is a busy, diverse role where you are guaranteed a varied, complex case load and plenty of opportunity to think outside the box.

Key responsibilities include:

- Developing coordinated care plans for LTS-CHC clients, to support an appropriate service response
- Completion of InterRAI assessments for adults aged 17-65 and Paediatric Needs Assessments for children aged 0-17 years
- Clear communication with clients and family/whanau to ensure their understanding of available support options and management of service expectations
- Communication with a diverse group of internal and external agencies, to facilitate appropriate support outcomes for clients and to provide advice on other appropriate resources
- Work closely with the Service Coordinator for LTS-CHC, to enable efficient workflow

**About You**

The attributes we seek include:
Essential
- At least 3 years experience working within healthcare
- A high level of skill with clinical assessment and critical evaluation
- Excellent interpersonal skills, with a high standard of verbal and written communication
- Ability to work autonomously, manage time well and prioritise workload appropriately
- Must be a health professional registered to work in New Zealand (Social work, Occupational Therapy, Physiotherapy, Registered Nurse or related discipline)
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth
- Full NZ drivers licence

Desirable
- Previous experience working within community-based health and disability services
- Have completed InterRAI Assessment training
- Positive approach to problem solving and developing innovative solutions
- Experience working in area of Mental Health and/or Alcohol and Drug Services
