**The Opportunity**
This is a 1-year fixed term position working 40 hours per week.

An exciting opportunity has arisen for an extraordinary nurse, providing specialist nursing care and expertise, both in direct care delivery and in support of other staff to support patients who are being worked up through our high suspicion of cancer / cancer pathways for our urology patients to increase timeliness to treatment & wellness through interactions and interventions to promote and facilitate faster access to cancer treatment / care.

As a highly experienced nurse with a wealth of nursing experience, you will have a warm, approachable manner, excellent communication skills across all population groups, which is supported by an understanding of Te Tiriti o Waitangi.

You will need to have, or develop, the clinical skills to provide support, enhance coordination, and collaboration of the multidisciplinary team within the Regional Cancer Treatment Service.

We are looking for a Clinical Nurse Coordinator - Urology with a passion for patient centred nursing care responsible for:

- Delivering key quality performance indicators associated with the role
- Ensure Faster Cancer Treatment and other national standards are met in a timely and efficient way
- Conduct clinic or telephone follow up of patients as per appropriate guidelines including establishing a nurse led follow up clinics
- Provide nurse led services for new and existing patients which includes follow up care for those recently discharged from hospital or referred for urgent assessment by intermediate care teams or primary care
- Liaise with in-patient teams to ensure that appropriate assessments and investigations for urology patients have been performed and that accurate clinical information is communicated back to primary care at discharge
- Contribute to the review and development of clinical patient pathways to ensure a standardised approach within urology
- Work proactively with other providers, including community teams, GP’s, social services, and palliative care to ensure seamless care and to reduce unplanned hospital admissions
