Overview

The Home Health Licensed Practical Nurse (LPN) is a licensed professional who administers personal care and routine skilled nursing care in the home to clients under the direction of a Registered Nurse. Collaborate with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to implement and follow a patient specific care and treatment plan. The LPN reports to the Nurse Manager.

Responsibilities
  1. Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique;
  2. Performs other duties as assigned;
  3. Implements plan of care initiated by the registered nurse;
  4. Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care;
  5. Provides accurate and timely documentation consistent with the plan of care;
  6. Participates in Case Conferences;
  7. Provides direct patient care as defined in the State Nurse Practice Act;
  8. Performs vital signs and head to toe patient assessment during visits;
  9. Manages and performs wound care treatment (including but not limited to surgical wounds, Wound Vac);
  10. Provides assessment, execution, and management of Central Line/Peripheral line, urinary catheters, colostomy, PleurX Drains, Lab Draws.
Qualifications
  • LPN Program Graduate
  • LPN - Licensed Practical Nurse
  • BLS - Basic Life Saving Certification
Competencies Skills
  • Clear Communication Skills Both Written And Verbal
  • Able To Keep Confidential Information Regarding Patients, Team Members
  • Able To Withstand Crisis Situations
  • Skills To Provide Customer Service To Patients, Team Members And Visitors
  • Proof Of Current/Valid Car Insurance
  • Knowledge And Experience With Electronic Health Records
  • Valid DL With No More Than 3 Moving Violations
Credentials
  • LPN - Licensed Practical Nurse
