Catholic Charities of Orange, Sullivan & Ulster is looking for a part-time Case Manager in their CHASSI program. CHASSI is the Children's Healthy and Social Safety Net Initiative.
The hourly rate for the position is $23.00/hour. This position does not come with benefits.
Summary:
This position provides direct case management services to the needy and disenfranchised in Orange and Sullivan County. It is expected that this will be accomplished through advocacy, providing information, referral, and short-term case management. The position will work in a collaborative manner connecting Social Safety Net Services with our Substance Use Disorder Departments and Prevention Department.
Essential Duties and Responsibilities include the following:
- Conduct initial intake utilizing Apricot platform database system.
- Actively participate in system-wide data sharing through integrated digital referral systems (Unite Us) or other means.
- Utilize basic Substance Use Disorder Assessments and brief bio-psychosocial intake to ensure all needs are identified comprehensively. Provide education to the client and family regarding adverse childhood effects and effects on the fetus when drug or alcohol use is present in the household, collaborating with the SUD clinician to provide comprehensive wrap-around services.
- Prioritize intakes and ensure warm handoffs throughout the CCOSU system and community partners, ensuring all intake and screening tools and notes are completed thoroughly and timely.
- Attend the Parent-Child Program training and develop service delivery with efficacy.
- Be part of an interdisciplinary treatment team to ensure all participants in the program have the full benefit of CCOSU resources.
- Maintain current and accurate documentation of services provided to clients.
- Answer telephones and provide needed information.
- Develop and maintain working relationships with providers in Orange and Sullivan Counties.
- Attend community and service provider meetings.
- Maintain an updated Community Resource list/guide to utilize as a referral source for clients who need them.
- Participate in outreach activities on weekends and after-hours such as community tabling events, pop-up pantry, food pantry outreach, and other case management activities.
- Provide community outreach to ensure access to the program and community education regarding services related to determinants of health.
- Participate in regular supervisory sessions with the immediate supervisor.
- Attend Agency in-service training, case conferences, and formalized coaching from the Parent-Child Program.
- Travel to meet clients at other CCOSU offices.
- Other duties as assigned by the supervisor or demands of the program.
Position Type and Expected Hours of Work:
This is a part-time position. The hours of work are 9:00 AM to 3:00 PM, Monday – Friday. This position requires the candidate to work 3 days per week or a total of 18 hours per week. We can be flexible with the weekly schedule, but it needs to be discussed prior to hiring and accepting the position. (It needs to be agreed upon by both parties). Reports to Monticello campus. This position requires the candidate to travel to multiple sites.

- Prepare comprehensive progress notes and service plans.
- Remain stationary at a workstation and use a computer at least 30% of the time.
Qualifications
Education and/or experience required:
- High School Diploma or Equivalency required with at least two years' experience working in case management with families.
- Associate's degree preferred.
- Must possess a clean NYS Driver's License and maintain it throughout employment.
Skills, Licenses, and/or competencies required:
- Knowledge of Social Services available in Orange and Sullivan counties – Preferred.
- Computer literate with knowledge of Microsoft Word, Excel, and Outlook.
- Excellent interpersonal and communication skills, work as part of a team.
- Crisis intervention skills and empathy in facilitating client service delivery.
- Good organizational, time management, and communication skills.
- Ability to demonstrate empathy and compassion while working with vulnerable clients.
- Individuals must possess the ability to work well independently as well as part of a team.
- Bilingual in English and Spanish.
