About The Role

An opportunity has arisen for an experienced and passionate Diabetes Nurse Specialist/Practitioner to join a dedicated multidisciplinary team. This role focuses on providing expert diabetes care, engaging with diverse communities, and contributing to improved health outcomes through a holistic approach.

Key Responsibilities
  • Provide expert diabetes management, including phlebotomy, medication titration, and tailored care plans
  • Prescribe medications and deliver timely, effective treatment
  • Administer vaccinations for clients with chronic conditions, particularly in community outreach settings
  • Engage with clients and whānau in their homes and communities to support care access and adherence
  • Collaborate within a multidisciplinary team to address complex health needs
  • Contribute to an innovative diabetes health programme supporting at-risk communities
Skills & Experience
  • Registered Nurse with a current Annual Practising Certificate (APC) and nurse prescriber/practitioner qualification
  • Proven experience in diabetes care, including medication titration and phlebotomy
  • Ability to administer vaccinations in community and outreach settings
  • Strong cultural competence, particularly in working with diverse communities
  • Excellent communication and organisational skills, with a proactive approach to client care
  • Ability to work collaboratively within a multidisciplinary team
