Te whiwhinga mahi | The Opportunity

The Intermediate Care service provides a range of responsive integrated services via a multidisciplinary team, to support people, in the community who are medically fragile with the following key principles:

  • Targeted at people who would otherwise face unnecessarily prolonged hospital stays or alternative admission to acute inpatient care, or long-term residential care
  • Services are provided based on a comprehensive assessment, resulting in a structured individual care plan that involves active therapy, treatment or opportunity for recovery
  • Services have a planned outcome of maximising independence
  • Intermediate Care includes the Hospital in the Home service working to support adults who have comorbidities or vulnerabilities including Heart Failure, Respiratory illness, Cellulitis and COVID19 to recover in their own homes


The Charge Nurse Manager is the front-line lead for this service and line manager for the clinical team including nursing and allied health staff.

You will be able to deal with challenging situations and changing environments demonstrating high level communication and leadership skills. You will work closely with the multi-disciplinary teams including the Clinical Charge Nurse and Practice Supervisor for Allied Health.

Nga Pūkenga Motuhake | Important Skills

  • New Zealand nursing registration, RGON, RN BHSc (Nursing)
  • Postgraduate certificate/ diploma in speciality nursing practice or working towards it
  • Minimum of five years' experience post registration
  • A commitment to achieving equitable outcomes for Māori
  • Highly skilled Leadership and change management experience
  • Experience working with and/or leading multidisciplinary teams
  • Extensive clinical experience/skills
  • Capacity to innovate and motivate your team
  • Strong, clear communication skills, with the ability to influence
This is a fixed term, full-time position and is based at Greenlane Clinical Centre
