The Hospital-in-the-Home (HITH) service has a new and exciting opportunity for an experienced Nurse Practitioner to join our team. You will receive SMO collegial support, and work alongside an experienced multidisciplinary team.


The
Hospital in the Home (HITH) is acute inpatient equivalent care, utilising highly skilled staff, hospital technologies, equipment, medication, and safety and quality standards, to deliver hospital-level care
within a person's place of residence or preferred (non-hospital) treatment location.


Patients' and their whānau are offered the choice to accept the very best care that is aligned to their values, needs and aspirations.

HITH delivers a platform for people to
maintain autonomy of their health and wellbeing, including acute care needs in their own home.

No two days will be the same. You will be conducting physical assessments, diagnosing, prescribing and managing patients with acute and sub-acute conditions, at risk of deterioration, within our virtual acute care ward in the home environment. Other facets of the role will see you undertaking telehealth, education, support of staff and an active role in service strategy and development.

Nga Pūkenga Motuhake | Important Skills
We are seeking a Nurse Practitioner with the following essential skills:

  • Skills in contemporary Acute and Emergency Medicine, General Medicine, Older People's Health, General Practice, or Urgent Care experience at an advanced practice level
  • Recent experience of managing complex medical patients
  • Confident in working autonomously
  • Registration with Nursing Council of New Zealand (NCNZ)
  • Registered with the nursing council NZ as a Nurse Practitioner with a minimum of 2 years of meeting nursing council requirements for a Nurse Practitioner
  • A commitment to achieving equitable outcomes for Māori
  • Demonstrate alignment with Te Toka Tumai values
