Our South West Community Team within Older Person - s Health and Rehabilitation provides assessment of elderly clients within the community to understand any support services or rehabilitation needs.

Our service is forward thinking, utilising InterRAI to effectively assess the needs of our clients.

We also have a restorative model of care and our work is focused on listening to our clients and helping to support and enable people to remain independent in their homes in the community or support their journey into residential care.


Our team of Clinical Assessors are integral to our interdisciplinary team and provide a key link to community services, visiting clients in their homes to assess their need for services in order to stay well in the community.

This is a 1 FTE permanent position, working Monday to Friday with specific hours to be negotiated.

This opportunity is open for an Allied Health Professional or a Registered Nurse to join our team.

Ko wai koe | About You

In this role, you will be responsible for:

  • Carrying out comprehensive assessment of clients medical, rehabilitation and support needs within their homes;
  • Establishing, coordinating and reviewing support plans;
  • Facilitating timely referrals to relevant service providers where appropriate;
  • Working collaboratively with the wider interdisciplinary team to ensure a holistic approach to service provision.

Ngā pūkenga me ngā wheako | Skills and experience

  • The ability to work autonomously, while also being able to work collaboratively within the interdisciplinary team;
  • Excellent interpersonal skills with the ability to build and nurture trusted relationship with clients and their whanau, as well as the interdisciplinary team and external community services;
  • Experience working within Older Person s Health is desirable;
  • Experience within a community setting is also advantageous;
  • Previous experience of the InterRAI is advantageous but not essential as full training will be provided;
  • You must be a qualified health professional;
  • As this is a communitybased position, a full NZ driver s license is essential.
