4 days per week,daytime hours; Mō te tūnga| About the role We areseeking a Clinical Nurse Specialist (Continence) to join our Older Persons Health and Rehabilitation Team.

This roleis diverse and encompasses promoting clinical excellence, undertakingspecialist care, nursing leadership, education, workforce development andworking alongside the wider team to promote continuous improvement andinnovation.

Ko waikoe | About You Clinical Nurse Specialists work across the organisation within a clinical specialtyacting in the roles of practitioner, educator, consultant, researcher,leader/change agent and care coordinator/case manager in the pursuit ofclinical excellence and improved health outcomes.

They are recognised as expertpractitioners, having in-depth knowledge of a speciality that results inexpanding boundaries of nursing practice and the implementation of evidence-basedpractice.

To besuccessful in this role, you will have passion and commitment for promotinghealth of older people and will have advanced nursing knowledge in continence,urology, gerontology, and chronic conditions.

You will be self-directed, canwork within a multidisciplinary environment, can cultivate relationships withteams/wider stakeholders and value contributing to positive workplace culture.

In thisrole, you will be responsible for: Providing advanced continence,urology and gerontology nursing knowledge and health assessment skills inrelation to the management of client care within the Older Persons Healthand Rehabilitation setting.

Role modelling expertevidence-based clinical practice.
Providing professionalnursing support to others in their assessment, clinical decision-making,implementation, evaluation, and documentation of care. Providing clinicalguidance and mentoring to nursing and allied health colleagues.
Guide, support, and actas a resource both internally and externally on the health of older adults. Network nationally toidentify and implement nursing advances and changes in practice.
Identify opportunitiesfor general health/wellness supports and initiatives for olderpopulations. Take a nursingleadership role in both direct and indirect client assessment and supportinterventions for patients and their whanau. Assist in facilitatinginterdisciplinary team meetings and provide professional support forcomplex clients with physical and social co-morbidities.

Provide nursingleadership and gerontology knowledge to a broad range of hospital, community,and professional groups to achieve positive outcomes for olderpopulations.

Participate in systemchange initiatives to improve health outcomes of older adults through evidence-basedpractice.

Participate in theplanning and delivery of the community nursing educational plan inconjunction with fellow nurse specialists and community leadership team.

Demonstrate ability tointerpret and implement research findings relevant to the health of olderpeople.
Supports a culture ofevaluation and the ongoing quality improvement of community nursingpractice. Participate in thedevelopment of and reviews current policy, procedures and standards usinga best practice approach.
Conduct, monitors andfacilitates clinical audit to ensure quality health outcomes. Be registered with the Nursing Council of New Zealand. Hold a current Nursing Council of New Zealand practising certificate. At least five yearsworking with older adults in a primary, community or secondary setting.
Experience with Continence and or urology Holds or is activelyworking towards a relevant Post-graduate Nursing Diploma.
Diploma to becompleted within an agreed timeframe.
Ideally including advanced healthassessment.
Ability to workautonomously with advanced nursing skills comparable to senior nurse orexpert PDRP level. Demonstrated skills innursing leadership and acts as a clinical role model for nurses. Displays a high degreeof confidence, communication skill, organisational skills, and personalmotivation. Knowledge and abilityto use computers and related technology efficiently.
Full driving licencewith no restrictions. Demonstrated ability todevelop nursing standards and quality initiatives.
Demonstrated teaching,mentoring and coaching skills. Demonstratedinvolvement in research that has changed nursing practice.
